By using a public Practice Care Plus or ConciergeOS surface, you agree to provide accurate operational information, use the service lawfully, and avoid submitting patient names, medical record numbers, diagnoses, medications, insurance identifiers, or clinical narratives to public forms.
SMS customer-care messages
Where a recipient affirmatively opts in, ConciergeOS may send recurring, non-clinical customer-care messages from or on behalf of the identified practice. Messages may include callback confirmations, appointment coordination, scheduling assistance, office-hour information, and responses to inbound inquiries. Message frequency varies. Message and data rates may apply. Consent is optional and is not required to receive healthcare services.
Reply STOP to opt out of SMS messages or HELP for help. Opting out of SMS does not delete an operational request or prevent other permitted contact through the practice. ConciergeOS does not send diagnoses, treatment details, medical records, or other sensitive health information by SMS.
Practice responsibility
Participating practices remain responsible for their legal identity, patient relationships, clinical decisions, appointment availability, consent practices, emergency instructions, and review of any communication before taking action. ConciergeOS does not guarantee a response, appointment, clinical outcome, or uninterrupted availability.
Privacy and data boundaries
Our Privacy Policy explains how operational information, mobile-number consent, and audit metadata are handled. Mobile numbers and SMS consent records are not sold, rented, or shared with third parties or affiliates for their own marketing or promotional purposes. Public forms are no-PHI surfaces.
